plan
- Employees can choose either the Basic or Enhanced Plan design
there are no medical questionnaires to complete
- if an employee's annual salary is greater than $45,000 they should enroll in the Enhanced
Plan, because of the higher disability income benefit
- mandatory participation of current and future employees working over 24 hours per
week.
- New employees are eligible for benefits after three months of full time employment
- employees covered by their spouse's plan for Extended Health and Dental can submit their
claims to both insurance companies to receive the allowable amount from both plans
- premiums can be shared 50/50 between the employer and employee
- the Employee Booklet has complete details Basic Plan (PDF), Enhanced Plan (PDF)
- all benefits will be effective the 1st of the month following receipt of your application and first
months premium
