• Any business with 3 or more employees, supplying the construction industry qualifies for this
  plan  
• Employees can choose either the Basic or Enhanced Plan design
  there are no medical questionnaires to complete  
• if an employee's annual salary is greater than $45,000 they should enroll in the Enhanced
  Plan, because of the higher disability income benefit 
• mandatory participation of current and future employees working over 24 hours per
  week.
• New employees are eligible for benefits after three months of full time employment 
• employees covered by their spouse's plan for Extended Health  and Dental can submit their
  claims to both insurance companies to receive the allowable amount from both plans   
• premiums can be shared  50/50 between the employer and employee
• the Employee Booklet has complete details Basic Plan (PDF),  Enhanced Plan (PDF)  
• all benefits will be effective the 1st of the month following receipt of your application and first
  months premium
