plan Anniversary is June 1, you will be notified of any plan changes 30 days prior
the Personal Benefit Selection is made for a 2 year period.
medical information is to be completed by all employees and their dependents at time of application.
if an employee has spousal Extended Medical and Dental coverage they can waive both or one of these benefits. If an employee wishes to Waive only one of these benefits you will have to contact us to obtain the rate
coverage begins the 1st of the month, following approval of your application
your premium is withdrawn from your account the 1st of each month, you will receive a statement each month
if you wish to cancel your plan you must give 30 days written notice
your Pay Direct Drug Card can be used to pay for paramedical services, if the practitioner is on line with the insurance company
