Things You Need to Know Owner Operator
Medical Evidence is required on employees, and their dependents
underwriting will take approximately 3 weeks, longer if medical information is missing or additional information has to be obtained from your droctor
new employees are eligible for benefits after three months of fulltime employment
mandatory participation of all employees working in excess of 24 hours per week
if an employee has spousal Extended Medical and Dental coverage they can waive both or one of these benefits
employer pays a minimum 50% of monthly premium
policy anniversary is April 1. You will receive 30 days notification advising you of any rate change
you can ,ake benefit changes at plan anniversary
we require 30 days written notice to cancel plan
