Things You Need to Know Owner Operator
  • Medical Evidence is required on employees, and their dependents
  • underwriting will take approximately 3 weeks, longer if medical information is missing or additional information has to be obtained from your droctor 
  • new employees are eligible for benefits after three months of fulltime employment 
  • mandatory participation of all employees working in excess of 24 hours per week
  • if an employee has spousal Extended Medical and Dental coverage they can waive both or one of these benefits  
  • employer pays a minimum 50% of monthly premium
  • policy anniversary is April 1. You will receive 30 days notification advising you of any rate change 
  • you can ,ake benefit changes at plan anniversary
  • we require 30 days written notice to cancel plan
